Team shirt order form

One person orders shirts for a group, with a size and a name for each shirt.

What's inside

Collecting a group shirt order

One parent often orders for three kids and themselves. Asking the size per shirt keeps a family's order in one reply instead of four.

Back names are optional per shirt, so a parent can name the kids' shirts and leave their own plain.

Close orders on a set day and say so in the intro. Most printers need a full size count before they start, and late orders mean a second print run.

Export the replies to a spreadsheet for the printer. Each shirt is a row with its size and name.

Shirt order questions

How do people pay for their shirts?

You collect payment outside the form, at practice or by a payment link you text after the order. Fillio takes no payment and no fee. At $18 a shirt, a parent ordering four owes $72, so put the shirt count next to the total in your text. Add a private note to each order once it's paid, before you send the size count to the printer.

Can I offer more colors or styles?

Yes. Add colors to the color question, or a style question for tees and hoodies. Ask style once per order unless people mix styles. Color is asked once per order too, so a family's shirts match. If hoodies cost more, put the price in the option label, such as Hoodie, $32, so parents know the total before they order.

Can it be used for a family reunion or a race?

Yes. Change the title, the intro and the colors. The per-shirt size and name work the same for a reunion, a camp or a charity run. For a race, put the race date and the pickup spot in the intro, and rename the practice option to packet pickup. For a reunion, keep shipping, since relatives often live far apart.
