Customer feedback form

A low score asks what went wrong, a high one asks for a review.

What's inside

One to three stars asks what went wrong and offers to make it right; four or five stars asks what they loved and for a public review.

Feedback you can act on the same day

Unhappy customers are asked what went wrong while it's fresh, and only those who want a reply leave a phone or email. Call them the same day.

Happy customers are the ones you ask for a review. Send the review link to everyone who said yes, while the visit is still in their head.

Put a QR code on the receipt, the table or the counter. Feedback given in the shop is more honest than an email a week later.

Customer feedback questions

Is the feedback anonymous?

Yes, unless the customer chooses to be contacted. The form asks for no name or email, and only a low-score customer who wants a reply leaves a way to reach them. Leaving names out gets you more honest answers about a cold coffee or a slow table. Say it in the intro, for example, anonymous unless you ask us to reply.

Why not ask everyone for a public review?

Asking only customers who gave four or five stars sends your happiest customers to review sites and brings problems to you first. Check each review site's rules, as some limit selective asking. Read the low scores every morning. A customer who gets a call within a day often comes back, and many never post the bad review at all.

Can I see my average rating?

Yes. Your responses show every rating, and the export gives you a column to average. Track it monthly to see whether a change worked. Wait for about 30 ratings before you trust the number. With two locations, duplicate the form per shop and print a separate QR code for each, so every location gets its own average.
