Fun run registration form
Runners pick a start wave and shirt size, and each wave fills to its limit.
What's inside
Each wave shows the places left and closes at 150; runners under 18 are asked for a parent or guardian, and the whole form closes once all three waves are full.

Planning a fun run that starts on time
Open registration six to eight weeks out and order shirts from the size answers two weeks before. Order 10 percent extra in M and L for race-day sign-ups.
Waves spread the start line: runners first, walkers and strollers last, 20 minutes apart. Set each wave's limit to what your start corral and the first kilometer can hold.
The acknowledgement is a plain sentence runners agree to. If your venue, insurer or school needs a formal waiver, paste their wording into the checkbox label instead.
Charging an entry fee or collecting donations? Put the amount and a payment link in the intro. To take only a set number of entries, see limited spots.
Fun run registration questions
Can one parent register a whole family?
This template takes one runner per reply, so bib numbers and shirt sizes stay one to one. Parents fill it in once per child, which takes under a minute each time. Ask families to use the same email for every runner, so you can find their entries together in the responses list. Remind them to pick the same wave, so a parent and child start together.
What happens when a wave is full?
The wave shows as full and runners pick another. Once all three waves hold 150, the form closes and shows that registration has ended. To keep places for race-day entries, set each wave's limit a little under your real number, for example 140. If you add a fourth wave, raise the form's total limit of 450 by the same amount.
Do I need a legal waiver for a fun run?
It depends on your venue and insurer, and this form is not legal advice. The template holds a simple risk acknowledgement; replace its wording with the text your insurer gives you. Check with them before registration opens, because runners who signed up earlier agreed to the old wording. For runners under 18, an insurer may want a parent or guardian to agree for them, and the form already asks for that name.
