Church connect card

First-time visitors say hello and choose how they'd like to get involved.

What's inside

First-time visitors are asked how they heard about the church; regulars skip that question.

Following up with visitors

Show a QR code on the screen during the welcome, and print it on the bulletin. Most visitors fill it in before the last song.

Email and phone are both optional. A visitor who leaves one is telling you how they want to hear from you, so use that one.

Reach out within the week, while the visit is fresh. A short note from a real person beats a newsletter sign-up.

The interests list routes each card. Kids' ministry goes to the children's pastor, serving to the volunteer lead, baptism to the pastor.

Connect card questions

Do visitors need an account to fill it in?

No. They scan the code or tap the link and answer in the browser. Only the church needs an account to see the cards and get an email when one arrives. Only the name and the visit question are required, so a shy first-timer can send just a name and still get a warm welcome that week.

Can regular members use the same card?

Yes. Members pick I come regularly and skip the first-visit question. Many churches use the card for prayer needs and serving sign-ups every week. Keep one link all year, so the bulletin code never changes. Read members' cards alongside the first-time ones, so a long-time member's need doesn't wait behind visitor follow-up.

Who sees the prayer request?

Only the people with access to your form's responses. If requests should go to a smaller prayer team, use a separate prayer request form with its own access. The prayer box is optional and comes last, so nobody feels they must share. Say in the intro who reads the cards, since people write more freely when they know.
